Polysaccharide Pneumococcal Vaccine

Prevention of Pneumococcal infections (infections of the ear, blood, brain and lungs)

✔️ Side Effects of Evimar 13

The most common adverse reactions include:

  • Injection site pain, tenderness (60%)
  • Swelling or induration at the injection site (20.3%)
  • Headache (17.6%)
  • Erythema at the injection site (16.4%)
  • Asthenia and fatigue (13.2%)
  • Myalgia (11.9%)
✔️ How does Evimar 13 work?

Evimar 13 Vaccine is a vaccine that helps develop immunity by initiating a mild infection. This type of infection does not cause illness but stimulates the body's immune system to produce antibodies (proteins) to protect against any future infections.

✔️ Quick Suggestions:
  • The Pneumococcal Polysaccharide Vaccine is given to prevent invasive diseases caused by pneumococcal bacteria such as pneumonia, meningitis, ear, and blood infections.
  • It is usually given as an injection into the muscle (intramuscular) of the upper arm.
  • Only one dose is needed for most people. A booster dose can be given 5 years after the first dose.
  • Inform your doctor if you have an infection with a high temperature, as vaccination may need to be delayed until recovery.
  • It may cause fever. Inform your doctor if it does not go away or if it gets higher than 102 degrees Fahrenheit.
✔️ Indications:

The 0.5 mL dose of the vaccine contains 25 micrograms of polysaccharide from each of the following 23 pneumococcal serotypes: 1, 2, 3, 4, 5, 6B, 7F, 8, 9N, 9V, 10A, 11A, 12F, 14, 15B, 17F, 18C, 19F, 19A, 20, 22F, 23F, and 33F. This vaccine is recommended for active immunization against pneumococcal disease in children aged 2 years and older, adolescents, and adults.

✔️ Pharmacology

The vaccine consists of purified pneumococcal capsular polysaccharide antigens from 23 serotypes, which cause around 90% of invasive pneumococcal diseases. The vaccine induces an immune response that helps prevent infection by these strains.

✔️ Dosage & Administration of Evimar 13

Primary vaccination (Adults and children 2 years and older):
A single dose of 0.5 mL via intramuscular or subcutaneous injection.

Special dosing considerations:

  • The vaccine should ideally be administered two weeks before elective splenectomy, chemotherapy, or immunosuppressive treatment. Avoid vaccination during chemotherapy or radiation therapy.
  • The vaccine should not be administered sooner than three months after completing chemotherapy or radiation therapy.
  • HIV-positive patients should be vaccinated soon after diagnosis.

Revaccination:

  • A single dose of 0.5 mL was administered intramuscularly or subcutaneously.
  • Revaccination should be based on official guidelines and not be done within three years due to increased risks of adverse reactions.
  • Revaccination may be considered after 5 years in individuals at increased risk, or after 3 years in high-risk populations like those with asplenia.

Note: The vaccine is not recommended for children under 2 years as their antibody response may be poor.

✔️ Interaction

The pneumococcal vaccine can be administered simultaneously with the influenza vaccine, provided separate needles and injection sites are used.

✔️ Contraindications

The vaccine should not be administered to individuals with:

  • Allergic reactions to any vaccine component.
  • Fever, acute infection, or chronic illness in the acute stage.
  • Uncontrolled epilepsy or other progressive neurological conditions.
✔️ Pregnancy & Lactation

Animal studies are insufficient to determine reproductive toxicity. Use during pregnancy only if necessary, and the potential benefits must justify the risks. It is unknown whether the vaccine is excreted in human milk, so caution is advised when administering it to breastfeeding mothers.

✔️ Precautions & Warnings
  • Delay vaccination in individuals with significant febrile illness or active infection unless delaying vaccination poses a greater risk.
  • The vaccine should not be injected intravascularly or intradermally, as these routes increase local reactions.
  • Patients receiving immunosuppressive therapy may have a reduced immune response and may not be fully protected.
  • Epinephrine should be readily available in case of an anaphylactic reaction.
  • The pneumococcal vaccine may not be effective in preventing infection in cases like basilar skull fractures or cerebrospinal fluid leakage.
✔️ Storage Conditions:

Store refrigerated at 2° to 8°C. Protect vials from light.

Frequently Asked Questions (FAQ)
